THE TEXAS GREENHOUSE PROJECT INDEPENDENT LIVING APPLICATION

THE PURPOSE of this application is to assess the eligibility and needs of LGBTQIA+ young adults between the ages of 19-24 aging out of foster care and group homes for enrollment in a Supervised Independent Living program, designed to facilitate their transition towards self-sufficiency and independent living.

DIRECTIONS: Please complete this application with accurate and detailed information to help us understand your needs and eligibility for the Supervised Independent Living program. Fill in all sections, providing explanations where necessary, and attach additional pages if more space is needed. It's important to answer all questions thoroughly, as this information will be critical in tailoring the program to support your successful transition to independent living.
